Proven and Emerging Best Practices in Social Determinants of Health
Centene and its subsidiaries have developed programs that address social determinants of health. This panel of experts will address barriers to healthcare through transformative pilot programs that test new models of care and implement evidence-based innovations to achieve sustainable health improvements for individuals, families, and communities.
This session will highlight:
Postpartum Pilot – This pilot identified, with health plan data, health disparities among cultural groups that were impacting member’s ability to attend timely postpartum visits.
Feeding America Partnership – Centene and Feeding America partnered to launch the ‘Food For Today and Food for Tomorrow’ resource development initiative, a program that will equip network
foodbanks and Centene health plans with best practice guidelines for meeting the needs of individuals experiencing food insecurity.
Rideshare –This pilot enables members to utilize ride-sharing solutions to get to and from medical appointments and other events.
