Proven and Emerging Best Practices in Social Determinants of Health
Centene and its subsidiaries have developed and implemented several programs that address social determinants of health. In this session, we will highlight the successes and obstacles of:
Rideshare – The rideshare pilot uses technology solutions to enable members to utilize ride-sharing solutions such as Lyft to get to and from medical appointments and other covered events.
Community Hub – Community care coordinators find at-risk individuals and connect them to the care they need. One health plan is reducing non-clinical barriers to care and improved birth outcomes of high-risk pregnancies.
Coordinated Entry through HMIS – To ensure chronically homeless members have access to all housing options, one health plan worked with its local partner to implement a centralized referral process that prioritizes available state-funded housing according to medical risk and necessity.
